DECEMBER - JUNE
To participate in activities offered through the Planning for Adult Life project click here http://www.PlanningforAdultLife.org
Identify the students vision for work and life you can use DDD's Person-Centered Planning Tool (PCPT) at http://www.nj.gov/humanservices/ddd/document/personcenteredplanningtool.doc
Research a list of DDD approved Support Coordination Agencies at: http://www.nj.gov/humanservices/ddd/services/supportcoordination.html
FEBRUARY - MARCH
Complete and submit the Support Coordination Agency Selection Form at: http://www.nj.gov/humanservices/ddd/services/supportcoordination.html
DDD will provide a Support Coordination Agency who can attend exit IEP and/or transition related meetings at the school and begin developing the Individualized Service Plan.
APRIL - JUNE
The ISP should be completed and approved prior to exiting the school system in order for services/supports to be available upon graduation.
