• REGISTRATION INFORMATION FOR ELEMENTARY AND HIGH SCHOOL STUDENTS

     
     
    Registration for any Pre-K thru 12th grade student will be done at the Keyport Board of Education Office
    370 Broad Street, Keyport, NJ

    Registration Requirements:
    The following information is needed when registering. The parent/guardian must come in person with the following documents:
    • Original Birth Certificate with raised seal
    • Immunizations records
    • 4 proofs of residency

    o   Utility Bills (gas, electric, etc)

    o   Mortgage Statement

    o   Municipal Tax Bill

    o   Signed (current) rental lease 

    • Student Transfer Card
    • Official Transcript from Previous School
    • Proof of Guardianship, if necessary
     
    Forms to be completed for registration:
    • Release of Records Form (to be picked up at registration office)
